Job summary

Circadia Health is seeking a Nurse Practitioner to join on-site at our partner facility in Clovis, CA. You’ll perform direct patient care, act on real-time predictive signals, and coach facility teams to translate data into earlier interventions.

This full‑time embedded NP role requires Monday–Friday on‑site presence, with responsibilities spanning assessments, care planning, staff training, and clinical workflow integration using Circadia’s AI tools.

Qualifications

Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
Eligible for Medicare participation; company bills on your behalf.
2+ years NP experience with complex adult populations; able to practice independently.
Valid CA driver’s license; comfortable traveling locally.
Thrives in a dynamic, evolving startup environment.
English required

Responsibilities

Direct patient care: initial and follow-up assessments; initiate/adjust plans of care; coordinate with facility providers.
Tech → bedside integration: train staff; standardize Circadia into clinical workflows.
Data into action: interpret trend‑based insights; prioritize high‑risk patients; drive timely interventions.
Close the loop: complete documentation within the same day.
High‑volume rounding: 30+ visits/day at SNFs; focus on new admits and Circadia alerts.
Quality & product feedback: surface field insights to improve workflows and tech integration.
Mentor & collaborate: support NP onboarding and partner with clinical ops/product teams.
