Circadia Health is hiring a Nurse Practitioner for an on-site, embedded role at a partner facility in Stoney Point, CA. You will deliver direct patient care, interpret predictive health signals, and train facility staff to integrate Circadia into daily workflows.

The position is full‑time with Monday–Friday on-site requirements; you will engage in hands‑on training, live demos, and shadowed assessments to drive early interventions and reduce readmissions.

Qualifications

• CA NP license and board certification required.
• 2+ years NP experience with complex adult populations.
• Ability to practice independently and travel locally as needed.
• Proficiency in English is required.

Responsabilités

• Provide direct patient care and follow‑up assessments for flagged residents.
• Train and coach facility staff to integrate Circadia into clinical workflows.
• Interpret trend insights and drive timely interventions to prevent decline.
• Complete notes and charts on the same day using AI‑powered tools.
• Conduct high‑volume visits at designated SNFs and support onboarding.
