Job summary

Circadia Health is seeking a full-time on-site Nurse Practitioner for our Lancaster, CA partner facility. You will see patients first, act on real-time AI health signals, and coach facility teams to turn data into early interventions.

This embedded role blends direct patient care with tech-to-bedside integration, requiring hands-on training, live demonstrations, and shadowed assessments. Expect high-volume rounds and collaboration with clinical ops and product teams.

Qualifications

• CA NP license and ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Minimum 2+ years NP experience with complex adult populations; able to practice independently.
• Eligible for Medicare participation; company bills on your behalf.
• Valid CA driver’s license; comfortable traveling locally.
• English required.
• Thrives in a dynamic startup environment.

Responsibilities

• Direct patient care: conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech to bedside integration: train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Close the loop: complete documentation/charts within the same day.
• High-volume rounding: efficiently complete visits across designated SNFs (typ. 25+/day), focusing on new admits, facility-flagged patients, and Circadia alerts.
• Quality & product feedback: surface field insights to improve workflows and tech integration in a clinical setting.
• Mentor & collaborate: support NP onboarding, share best practices, and partner with clinical ops/product teams.
