Job summary

Circadia Health is seeking an on-site Nurse Practitioner to join our embedded role at Mirage Post-Acute in Lancaster, CA. You will perform initial and ongoing patient assessments, coordinate with facility teams, and integrate Circadia’s AI-powered workflow into daily care. This is a full-time on-site position with travel to partner facilities as needed.

You will lead training for staff, translate data signals into action, and ensure timely documentation. English required; Spanish a plus.

Qualifications

• Nurse Practitioner license with ANCC or equivalent required.
• 2+ years NP experience with complex adult populations.
• Eligible for Medicare participation; can practice independently.
• English required; Spanish a plus.

Responsibilities

• Direct patient care: initial and follow-up assessments; plan of care; consent; coordination with facility providers.
• Tech → bedside integration: train staff to embed Circadia into clinical workflows.
• Data into action: interpret trend-based insights and drive timely interventions.
• Close the loop: complete documentation within the same day.
• High-volume rounding: ~25+ visits/day focusing on new admissions and flagged residents.
