Job summary

Circadia Health is seeking a licensed Nurse Practitioner for an on-site, embedded role at a partner facility in Los Altos, CA. You will perform initial and follow-up assessments, initiate care plans, and coordinate with facility providers while training staff to integrate Circadia into daily workflows.

Responsibilities include interpreting trend-based health signals, prioritizing high-risk residents, and documenting notes within 24 hours.

Qualifications

• CA license and NP certification required.
• np certification in CA (ANCC or equivalent) preferred.
• 2+ years NP experience with complex adult populations.
• English fluency required; Spanish helpful.

Responsibilities

• Provide initial and follow-up assessments; update plans of care.
• Train staff to integrate Circadia into clinical workflows.
• Interpret trend-based insights and prioritize high-risk patients.
• Complete notes within 24 hours of each encounter.
• Conduct high-volume visits (30+ per day) focusing on new admissions.
• Provide feedback to improve workflows and tech usage.
• Mentor NP onboarding and collaborate with clinical ops teams.
