Qualifications

Responsibilities

• Conduct daily wellbeing checks and support medical intake under supervision.
• Schedule and coordinate medical and behavioral health appointments and follow-ups.
• Assist clients with medication self-administration and monitor adherence.
• Document health interactions, logs, and care coordination in EHR promptly.
• Provide basic health education on adherence, hygiene, and preventive care.
• Report changes in client condition to Medical Coordinator or supervisor.
• Participate in daily huddles and IDT meetings for integrated care.
• Maintain a safe, clean environment and assist with infection control.
• Collaborate with case managers and housing teams to support recovery.
• Ensure HIPAA/OSHA compliance and program policy adherence.
• Track program metrics and client health outcomes as directed.
