Job summary

Whole Person Care Clinic (WPCC) is seeking a Medical Coordinator Assistant to support health monitoring, medication self-administration, and care coordination for Recuperative Care clients. You will work under the Medical Coordinator and clinical team to ensure accurate documentation in the EHR and a trauma-informed environment for clients recovering from illness or homelessness.

The role requires CMA/RMA/EMT certification, 1 year of relevant experience, and strong communication.

Qualifications

• High School Diploma or GED required.
• CMA, RMA, or EMT certification required.
• Minimum 1 year of experience in a clinical, shelter, or social services setting.
• Experience with homelessness, chronic illness, or behavioral health preferred.
• Familiarity with EHR systems and documentation.

Responsibilities

• Conduct daily well-being checks and support medical intake under supervision.
• Coordinate and schedule medical and behavioral health appointments.
• Support medication self-administration and monitor adherence.
• Document health interactions and care coordination in the EHR.
• Provide basic health education on adherence, hygiene, and prevention.
• Communicate changes in condition or plan adherence promptly.
• Participate in daily huddles and IDT meetings for integrated care.
• Maintain a safe, clean environment and assist with infection control.
• Collaborate with case managers, behavioral health staff, and housing teams.
• Ensure HIPAA, OSHA, and WPCC policy compliance.
• Assist with tracking program metrics and client health outcomes.
