Job summary

Whole Person Care Clinic is seeking a Medical Coordinator Assistant to support client health, care coordination, and documentation for WPCC programs, including Recuperative Care. This role operates under supervision of authorized clinical staff to help health stabilization and continuity of care.

You will assist with scheduling and coordinating medical, behavioral health, and specialty appointments; monitor client conditions, provide basic health education, and maintain accurate logs.

Qualifications

• High school diploma or equivalent required.
• Current EMT Certification required.
• BLS certification required.

Responsibilities

• Assist with client health support and care coordination under authorized supervision.
• Document health-related interactions and coordination activities in the EHR accurately and timely.
• Schedule and coordinate medical, behavioral health, and specialty appointments.
• Participate in daily huddles and interdisciplinary team meetings as assigned.
• Collaborate with Case Managers, housing staff, and clinical staff to support integrated care.
