• Requires CMA/RMA or EMT certification and a HS diploma.
• Experience with EHR systems and multidisciplinary care support is preferred.

Responsibilities

• Perform daily client well-being checks under supervision.
• Assist in scheduling medical/behavioral health appointments and follow-ups.
• Support medication self-administration and monitor adherence.
• Document health interactions and care coordination in the EHR.
• Provide basic health education on prevention and hygiene.
