Qualifications

• Bachelor's Degree in social services or related field with minimum one year experience coordinating supportive housing or independent living services.
• High School Diploma or GED with two years of experience coordinating supportive housing or independent living services.

Responsibilities

• Conduct assessments and interviews with clients to determine eligibility.
• Develop and update action plans and case notes for client files.
• Provide ongoing life skills development to improve independence and quality of life.
• Document care and follow support plans with strong written communication.
• Coordinate referrals to medical, mental health, and social services.
• Travel throughout the area to assist clients and conduct visits as needed.
