Compass Career Solutions seeks a housing program coordinator to perform client assessments, develop action plans, and provide life skills coaching. The role requires in-person work and community outreach with potential eligibility interviews and coordination with medical and social services.

Training and orientation are provided. The position emphasizes documentation, collaboration with teams, and ensuring clients access SSI/SSDI, Medicaid, and related supports while traveling within the service

Qualifikationen

• Bachelor's Degree in social services or related field with minimum one year experience coordinating supportive housing or independent living services.
• High School Diploma or GED with two years of relevant experience in housing/coordinating living services.

Aufgaben

• Conduct assessments and interviews to determine eligibility.
• Develop and update action plans, case notes, and client files.
• Provide ongoing life skills development to improve independence and quality of life.
• Document care and follow support plans with strong reading and writing communication.
• Coordinate referrals to medical, mental health, and other services.
• Assist SSI/SSDI/Medicaid/Medicare/SNAP applications and redeterminations.
