Job summary

Hamilton Health Box is seeking a Licensed Practical Nurse (LPN) to serve as On-Site Clinician Assistant (OSC) across locations. The OSC handles clinical and administrative duties, including scheduling, care delivery, health education, and care coordination with pharmacy and referrals.

Role requires LPN license, BLS on onboarding, and bilingual English/Spanish proficiency. Travel between clinics may be required; EMR systems usage is expected.

Qualifications

• Licensed Practical Nurse license required.
• BLS certification prior to onboarding (AHA or Red Cross).
• Bilingual in English and Spanish preferred.

Responsibilities

• Collaborate with departments to ensure employees receive appropriate care and programs.
• Formulate individualized programs based on health risk assessment results.
• Assist with annual physicals and screenings, testing, and results reporting.
• Coordinate client referrals and document progress in medical records.
• Travel between clinics as needed.
