Hamilton Health Box is hiring an On-Site Clinician Assistant (OSC) to support clinical and administrative operations at local clinics. As an LPN/LVN, you will interact with patients for scheduling and follow-up, deliver protocol-driven care, and assist with health education and preventative initiatives.

You will coordinate with providers, manage EMR entries, and help maintain strong community relations while traveling between local clinics as needed.

Qualifikationen

• LPN/LVN license required to practice.
• BLS for Healthcare Providers required prior to onboarding.
• Bilingual in English and Spanish to communicate with patients.

• Collaborate with departments to ensure employee care and programs.
• Coordinate health screenings, annual physicals and follow-up exams.
• Register patients in EMR and document visits and education.
• Schedule follow-up exams and consultations; manage referrals.
• Support health education and wellness initiatives for staff.
