Job summary

Hamilton Health Box is seeking a Licensed Practical Nurse (LPN/LVN) to serve as an On-Site Clinician Assistant (OSC) at our locations. The OSC handles daily clinical and administrative tasks, greets patients, schedules follow-ups, administers protocol-driven care, and coordinates with providers and pharmacy services.

Travel to local sites may be required. The role emphasizes high-touch, low-cost care with a focus on patient education, preventative screening, and care navigation.

Qualifications

• Licensed Practical Nurse (LPN/LVN) with current license.
• BLS Certification is required prior to onboarding.
• Fluency in English and Spanish preferred for patient communication.

Responsibilities

• Collaborate with departments to ensure employees receive appropriate care and programs.
• Formulate individualized programs based on health risk assessments.
• Assist with annual physicals, screenings, and follow-up exams.
• Schedule and complete follow-up exams and consultations.
• Coordinate referrals and document progress in medical records.
• Lead health education sessions and health fairs for employees.
• Collect and interpret utilization data with timely documentation.
• Provide coaching and interpretation of screening results to clients.
