Travel to nearby sites may be required as HHB scales its telemedicine-enabled clinics. The role emphasizes collaboration with providers, patient education, and care navigation within electronic medical records.

Qualifications

• > Licensed Practical Nurse with current license.
• "> BLS for Healthcare Providers before onboarding.
• "> Bilingual in English and Spanish preferred.

Responsibilities

• Collaborate with departments to provide care and programs.
• Develop individualized health programs based on risk assessments.
• Assist with annual physicals and screenings; document results.
• Schedule follow-ups, exams, and return-to-work assessments.
• Coordinate referrals and document progress in medical records.
• Lead health fairs and group trainings; ensure regulatory compliance.
• Collect data on program utilization and outcomes; maintain records.
