Hamilton Health Box, Inc. is seeking a Licensed Practical Nurse (LPN/LVN) to serve as an On-Site Clinician Assistant in Clinton, OK. The OSC will manage daily clinical and administrative operations, deliver protocol-driven care, schedule follow-ups, and coordinate care with providers and pharmacy services.

The role requires BLS, strong communication, and health-education capabilities. Travel to local locations may be required; bilingual English/Spanish is preferred.

Qualifications

• Licensed Practical Nurse (LPN) qualification required.
• BLS certification required prior to onboarding (American Heart Association or Red Cross).
• Ability to communicate effectively at all levels within an organization.
• Knowledge of disease and lifestyle topics such as smoking cessation, weight management, nutrition, and chronic conditions.

Responsabilités

• Collaborate with departments to ensure employee care and programs.
• Formulate individualized programs based on health risk assessments.
• Assist with annual physicals, screenings, and follow-up exams.
• Schedule, complete follow-up exams, and return-to-work evaluations.
• Coordinate referrals and document progress in medical records.
• Lead health education sessions and coordinate health fairs.
