Job summary

VOALA is seeking a Case Manager (Specialist) to provide intensive case management to eligible clients, meeting participants at least weekly. This on-site role coordinates services and care for a diverse caseload and acts as an advocate within community systems.

Qualifications include an Associate’s Degree or equivalent, at least 1 year in case management, and completion of VOALA Case Management training within 6 months. Salary ranges from $23.98 to $25.24 per hour with competitive benefits.

Qualifications

• Associate’s Degree in Social Work or related field required or equivalent combination of education and experience.
• Completion of VOALA Case Management training within 6 months of hire.
• Minimum 1-year general experience in case management or similar social services role.

Responsibilities

• Conducts comprehensive screenings and assessments to collect information for a case plan.
• Provide service linkages and support systems to meet identified needs.
• Coordinates planning with clients to meet short- and long-term goals.
• Coordinate care and case management of assigned caseload within the team.

Education

Associate’s Degree in Social Work or related field

Combination of education and relevant experience (Two years’ total)
