Job summary

VOALA in Los Angeles area seeks a dedicated Case Manager (Specialist) to provide intensive case management services, meeting participants weekly and coordinating services. You will assess needs, develop action plans, connect clients to resources, and advocate on their behalf before agencies and community bodies.

• Ability to perform comprehensive screenings and assessments to develop a case plan.
• Proven ability to coordinate services and resources for diverse client needs.
• Experience coordinating care and advocating with external agencies.

Responsibilities

• Conducts comprehensive screenings and assessments to develop a case plan.
• Provide service linkages and support systems to meet identified needs.
• Coordinates individualized planning with clients to meet short- and long-term needs.
• Advocacy for participants before agencies and community settings.
• Capacity Building – motivating participant progress.
• Outreach and engagement to bring participants into programs.
