Job summary

Volunteers of America Los Angeles (VOALA) is seeking a Case Manager (Specialist) to provide intensive case management to eligible clients, meeting participants weekly and coordinating services across housing, health, and social supports.

The role coordinates care for a diverse caseload and requires on-site work daily; applicants must meet education requirements and complete VOALA training within 6 months. VOALA offers medical, dental, vision, and retirement benefits.

Qualifications

• Must pass fingerprint clearance and background checks.
• Ability to work under emotional stress and with challenging clients.
• Experience with homelessness, mental illness, substance abuse, or re-entry challenges.
• Associate’s Degree in Social Work or related field or equivalent experience.
• Completion of VOALA Case Management training within 6 months of hire.
• Minimum 1 year of general case management or similar social services experience.

Responsibilities

• Conducts comprehensive screenings and assessments to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information to develop a case plan.
• Provide service linkages and support systems to ensure identified needs are met.
• Coordinate individualized planning with clients to meet short- and long-term needs.
• Coordinate case conferences with residents, consults with service team, monitor progress of clients on case plan, assist clients to meet goals.
• Advocate for clients before other agencies and community bodies as needed.
• Facilitate group education sessions and document sessions clearly and concisely.
