Job summary

LHH is seeking a compassionate Case Manager to support a senior housing community in Buellton, CA. This hands-on, on-site role directly impacts residents' ability to be safely housed.

You will deliver strengths-based case management, connect residents to essential resources, and collaborate with outside partners to build supportive networks. The role emphasizes independence with teamwork.

Qualifications

• Bachelor’s degree required; Master’s in social work or related field preferred.
• At least 3 years of case management or related social services experience.
• Experience with seniors or formerly unhoused populations preferred.
• Ability to work independently while collaborating with external partners.
• Strong communication, organization, and follow-through skills.
• Spanish-speaking is a plus but not required.

Responsibilities

• Deliver individualized, strengths-based case management grounded in residents’ needs.
• Connect residents to community resources (mental health care, financial assistance, senior services, veteran supports).
• Partner with outside caseworkers, agencies, and community organizations to build strong support networks.
• Support transitions into housing after homelessness.
• Help navigate health, housing stability, and organization/hoarding challenges with compassion.
• Foster connection through on-site activities and wellness events.
