Job summary

The People Concern in Los Angeles is seeking a Case Manager to provide intensive support to Clients facing homelessness. This role includes maintaining Client relationships and managing a caseload with a focus on achieving housing stability through evidence-based practices.

As a Case Manager, you will collaborate with Clients, assist them in overcoming barriers to housing, and guide them towards self-sufficiency. Candidates should have a High School Diploma or GED and a valid driver's license, with a preference for those having case management experience. Full-time hours are Monday to Friday.

Qualifications

• Must be willing to transport Clients in personal vehicle.
• Able to work in a high tolerance Harm Reduction model.
• Experience in case management is a plus.

Responsibilities

• Maintain a caseload of approximately 20 high-acuity Clients.
• Develop relationships with Clients focusing on independence.
• Write and maintain accurate progress notes in databases.
• Support Clients by teaching life skills like budgeting.
• Coordinate access to health care and social services.
