Job summary

LHH is seeking a compassionate Case Manager to support a senior housing community in Buellton, CA. This hands-on, on-site role directly impacts residents' ability to be safely housed.

You will deliver strengths-based case management, connect residents to essential resources, and collaborate with outside partners to build supportive networks. The role emphasizes independence with teamwork.

Qualifications

  • Bachelor’s degree required; Master’s in social work or related field preferred.
  • At least 3 years of case management or related social services experience.
  • Experience with seniors or formerly unhoused populations preferred.
  • Ability to work independently while collaborating with external partners.
  • Strong communication, organization, and follow-through skills.
  • Spanish-speaking is a plus but not required.

Job description

We’re looking for a compassionate, resourceful Case Manager to support a senior housing community in Buellton, CA. This is a hands-on, on-site role where your work directly impacts people’s ability to remain safely and successfully housed.

What You’ll Be Doing

  • Deliver individualized, strengths‑based case management grounded in each resident’s unique needs
  • Connect residents to critical community resources (mental health care, financial assistance, senior services, veteran supports, and more)
  • Partner closely with outside caseworkers, agencies, and community organizations to build strong support networks
  • Support residents as they successfully transition into housing after homelessness
  • Help individuals navigate challenges related to health, housing stability, and organization/hoarding concerns with compassion and respect
  • Foster connection and belonging by supporting on‑site community activities like meetings, groups, gardens, classes, and wellness events

What We’re Looking For

  • Bachelor’s degree (required) or Master’s degree (MSW, MFT, or related, preferred)
  • A minimum of 3 years of case management or related social services experience (depending on education level)
  • Experience working with seniors, formerly unhoused individuals, or high-acuity populations is strongly preferred
  • Comfortable working independently while collaborating with external partners
  • Strong communication, organization, and follow-through skills
  • Spanish speaking is a plus, but not required
