Experience working with vulnerable populations, especially those with disabilities.
Responsibilities
Conduct initial and quarterly psychosocial assessments.
Collaborate with participants on individualized service plans.
Build trusted relationships with clients.
Provide outreach, housing navigation, and ongoing support.
Facilitate referrals to healthcare and community services.
Support clients in obtaining employment and education opportunities.
