Responsibilities

• Conduct initial and quarterly psychosocial assessments.
• Collaborate with participants on individualized service plans.
• Build trusted relationships with clients.
• Provide outreach, housing navigation, and ongoing support.
• Facilitate referrals to healthcare and community services.
• Support clients in obtaining employment and education opportunities.
