Job summary

MidPen Housing Corp. in Santa Cruz, CA seeks a Case Manager I to engage supportive housing residents, help maintain housing, and support goals leading to independent living.

The role requires an MSW/MA or BA with related experience, knowledge of trauma-informed care, and travel between properties. You will conduct outreach, provide crisis support, and coordinate services with partners to help residents thrive.

Qualifications

• MSW or MA in psychology/counseling, or BA/BS with relevant experience
• Experience serving high-need populations, including homelessness
• Experience with trauma-informed care and harm reduction
• Ability to travel between properties
• Experience using assessment tools and interpreting data

Responsibilities

• Deliver onsite programming at assigned properties, ensuring alignment with standards
• Outreach via door-to-door, phone, email to promote services
• Provide crisis support and follow mandated reporting procedures
• Support residents with independent living skills
• Facilitate group and individual services and promote engagement
• Collaborate with third-party partners and local agencies
• Maintain records, ensure timely reporting, HIPAA compliance
• Assess resident needs with empathy and professionalism
• Participate in team meetings and maintain relationships with staff
