Job summary

VOALA is seeking a Care Manager (Generalist) to provide intensive case management for participants with high acuity, including involvement with the criminal justice system and co-occurring disorders. On-site work, weekly contacts, and coordinated service linkages are core to this role.

Responsibilities include comprehensive assessments, treatment planning, crisis intervention, advocacy in court and community settings, and maintaining confidential records in line with HIPAA and funder

Qualifications

• Bachelor’s Degree in Social Work or related field, or 4 years’ combined education/experience.
• At least 1 year of case management experience in homelessness or re-entry programs.
• Pass fingerprint and LASD background checks.
• Valid CA driver’s license and reliable transportation.
• Ability to handle high-acuity clients and emotional stress.
• Pass DHS approval.

Responsibilities

• Conduct comprehensive screenings and assessments to develop action plans and meet client needs.
• Coordinate care and service linkages for clients.
• Meet with clients regularly to track progress and adjust plans.
• Maintain confidential records per HIPAA and funder requirements.
• Advocate for clients in court and community settings.
• Lead group education sessions and support groups.
• On-site service delivery every day of active employment.
