Job summary

Volunteers of America Los Angeles is seeking a Care Manager (Generalist) to provide intensive, on-site case management for participants with high acuity and co-occurring disorders. You will assess needs, coordinate services, and develop individualized care plans to support successful community reintegration.

The role requires HIPAA-compliant record-keeping, strong collaboration with service providers, and advocacy in court or agency settings as needed. Competitive pay and benefits offered.

Qualifications

• Bachelor’s degree in Social Work or related field is preferred.
• At least 1 year case management experience in homelessness, addictions, or re-entry programs.
• Must pass fingerprint clearance and background checks; valid CA driver's license.

Responsibilities

• Provide intensive care coordination for participants with high acuity and co-occurring disorders.
• Develop detailed care plans, set goals, and connect clients to required services.
• Coordinate with service providers; maintain records with HIPAA compliance.
• Lead support groups and advocacy activities; attend relevant meetings and court appearances as needed.
• Maintain confidential client data and complete progress notes in a timely manner.
• Report to Program Manager and participate in team-based care planning.
