On-site position requiring weekly contact with clients, collaboration with a service team, and thorough documentation in HIPAA-compliant records.

Qualifikationen

• Bachelor’s Degree in Social Work or related field.
• Total of 5 years education/experience related to homelessness, addictions, prison re-entry program care.
• Pass fingerprint clearance and LASD background check.
• Valid California driver’s license and reliable transportation.

• Conducts comprehensive screenings and assessments to collect information and develop detailed plans to meet high acuity needs.
• Provide service linkages and support systems to ensure identified needs are met.
• Coordinates individualized planning and follow-up with clients to meet short and long-term goals; tracks progress.
• Maintains confidential client data per HIPAA and other regulations; records services and case notes.
• Advocates for clients in court and before agencies/community settings; facilitates group sessions.
