Volunteers of America Los Angeles (VOALA) is hiring a Clinical Case Manager I to provide intensive case management services to clients with multiple co-morbidities, helping them access counseling, planning and crisis intervention as they re-enter society. This on-site role requires engaging with individuals and families, coordinating care, and advocating across agency and community partners.

Minimum qualifications include a Master’s degree in social work, psychology, or counseling, and

Qualifications

• Fingerprint clearance and background check required.
• Valid California driver’s license and automobile insurance.
• Experience in human services for a special needs population (1 year).
• E‑Verify employment eligibility.

Responsabilités

• Conducts comprehensive screenings and assessments to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information.
• Assess barriers facing clients and develop a case plan.
• Coordinate individualized planning with clients to meet short- and long‑term needs.
• Provide service linkages and support systems to ensure identified needs are met.
• Coordinate case conferences with clients and monitor progress of case plans.

Formation

Master’s degree in social work, psychology, or counseling

Registered with the Board of Behavioral Sciences as an ASW or AMFT
