Job summary

Via Care Community Health Center in Los Angeles, CA is seeking a Justice-Involved Case Manager (Lived Experience Role) to support patients transitioning from correctional settings. The role leverages lived experience to build trust and assist with care planning, referrals, and education.

Qualifications

• Lived experience with incarceration is strongly preferred and valued.
• Case Management certificate or related experience is required.
• Medical Assistant training/certification is expected.
• Minimum of one year experience in the medical field is required.
• CPR certification for adult, child, and infant is required.
• Experience with electronic health records is desirable.

Responsibilities

• Discharge and arrange referrals or follow-up appointments.
• Act as patient advocate and liaison for providers and social services.
• Schedule continuity of care and other referrals for patients.
• Provide health education to patients per provider request.
• Maintain patient records and ensure accurate documentation.
• Assist with immunization inventory and lab ordering as directed.
• Support front office duties and attend team meetings.
