Job summary

Volunteers of America Los Angeles (VOALA) is seeking a dedicated Care Manager (Generalist) to provide intensive care coordination for participants involved with the criminal justice system and facing co-occurring disorders.

This on-site role requires screening, service linkages, treatment planning, court advocacy, and collaborative work with a service team to help clients achieve goals and safer reentry.

Qualifications

• Bachelor’s degree in social work or related field required.
• Minimum 1 year of case management experience in relevant settings; clinical or re-entry focus preferred.
• Ability to coordinate complex services, advocate for clients, and maintain confidentiality under HIPAA.

Responsibilities

• Conduct comprehensive assessments to identify client needs, plan actions, and coordinate resources.
• Provide service linkages and support to ensure identified needs are met.
• Coordinate care plans with clients and service providers, tracking progress toward goals.
• Maintain case notes and documentation in accordance with HIPAA and funder requirements.
• Facilitate group sessions and advocacy activities to support client re-entry and adjustment.
