Job summary

Community Options, Inc. in Lubbock, TX seeks an experienced Provider Service Coordinator to assess needs and coordinate supports for individuals with disabilities. The role covers ISP/PCP development, service placement, and collaboration with families and care teams.

The PSC will advocate for clients, coordinate services, ensure accurate documentation, and participate in planning teams. On-call availability is required to support placement and transition processes.

Qualifications

• Demonstrates strong communication and problem-solving in supporting individuals with disabilities.
• Experience coordinating services with interdisciplinary teams.
• Ability to develop and adjust ISP/PCP with input from families and staff.

Responsibilities

• Advocate for the needs and choices of individuals in our care.
• Assist with determining appropriate support services during placement and transition.
• Develop and update ISP/PCP with an interdisciplinary team, including concerns and behavioral developments.
• Develop and monitor training goals aligned with ISP/PCP.
• Communicate with families and guardians as needed.
• Identify, develop, and coordinate community resources and support.
• Submit documentation to maintain eligibility for government programs.
• Ensure program documentation and billable records are completed accurately and timely.
• Serve as an active member of each individual’s planning team.
• May perform the role of direct support professional as needed.
• Cooperate with license and department staff during inspections or inquiries.
