Community Options, Inc. seeks an experienced Provider Service Coordinator for an on-call role in Lubbock, TX. You will assess needs, identify supports, and coordinate services to promote development of physical, intellectual, emotional, and social skills.

The PSC will develop and monitor ISP/PCP with an interdisciplinary team, advocate for individuals, and coordinate community resources. Requires a strong background in supporting individuals with developmental disabilities and a valid driver’s

Qualifications

• Team-oriented with demonstrated leadership experience.
• Experience solving problems across multiple priorities.
• Ability to establish a supportive relationship with individuals receiving supports.
• Bachelor’s degree preferred; may be required in some states.

Responsabilités

• Advocate for the needs and choices of individuals in our care.
• Coordinate and monitor ISP/PCP with an interdisciplinary team.
• Develop and update ISP/PCP with concerns, observations, and behavioral developments.
• Develop training goals aligned with the ISP/PCP.
• Communicate with families and guardians as needed.
• Identify, develop, and coordinate community resources and support.
• Submit required documentation to maintain eligibility for government programs.
• Ensure program documentation and billable records are accurate and timely.
• Serve as an active member of each individual’s planning team.
• May perform the role of direct support professional when staffing needs arise.
• Cooperate with license and department staff during inspections.
