Job summary

Community Options, Inc. in Dallas, TX is seeking a Provider Service Coordinator to assess the needs of individuals with intellectual and developmental disabilities and coordinate support services.

The PSC will develop ISP/PCP with an interdisciplinary team, advocate for clients, coordinate resources, and ensure documentation and eligibility for government programs. Bachelor’s degree is preferred; valid driver’s license required; crisis-management skills are essential.

Qualifications

• High School Diploma or GED with relevant experience in disabilities services.
• Bachelor’s Degree preferred; may be required in some states.
• Valid driver’s license with a satisfactory driving record.
• Team leadership and collaborative problem-solving abilities.
• Ability to build supportive relationships with individuals receiving supports.
• Ability to remain calm and exercise good judgment in crises.

Responsibilities

• Advocate for the needs and choices of individuals in our care.
• Assist with determining appropriate support services during placement and transition.
• Develop and update ISP/PCP with an interdisciplinary team including concerns and progress.
• Develop and monitor training goals aligned with the ISP/PCP.
• Communicate with families and guardians as needed.
• Identify, develop, and coordinate community resources and support.
• Initiate level of care assessments when an individual's ability changes.
• Submit required documentation to maintain eligibility for government assistance programs.
• Ensure program documentation and billable records are accurate and timely.
• Serve as an active member on each individual’s planning team.
• Cooperate with the licensee and department staff during inspections or inquiries.
• May perform the role of Direct Support Professional when staffing needs arise.
• Additional tasks and responsibilities may be assigned.
