Job summary

Community Options, Inc. is seeking a Provider Service Coordinator in San Antonio, TX. The PSC assesses needs of individuals with intellectual and developmental disabilities and coordinates supports to promote learning and development across physical, intellectual, emotional, and social skills.

The role offers clear responsibilities for advocating, developing ISP/PCP plans, coordinating resources, and ensuring regulatory documentation is maintained with a strong emphasis on teamwork and safety.

Qualifications

• High school diploma or GED with two years of experience supporting individuals with developmental disabilities.
• Bachelor’s degree preferred; may be required in some states.

Responsibilities

• Advocate for the needs and choices of individuals in our care.
• Assist with determining appropriate support services during the placement and transition process.
• Develop and update ISP/PCP with an interdisciplinary team to include concerns and behavioral developments.
• Develop and monitor training goals aligned with the ISP/PCP.
• Communicate with families and guardians as needed.
• Identify, develop, and coordinate community resources and support.
• Submit required documentation to maintain eligibility for government assistance programs.
• Ensure program documentation and billable records are completed accurately and timely.
• Serve as an active member on each individual’s planning team.
• May perform the role of direct support professional as necessary when staffing needs arise.
