Job summary

All Care is seeking an On-Call Hospice RN to provide after-hours nursing support in patients' homes across Boston and surrounding communities. You will triage calls, manage urgent symptoms, perform visits as needed, and ensure continuity of care per the plan of care and hospice standards.

The role requires a MA RN license, a valid driver’s license, and 1–2 years of clinical experience (hospice, ICU, or ED preferred).

Qualifications

• Active Massachusetts Registered Nurse license required.
• Valid driver’s license and reliable transportation.
• 1–2 years recent clinical experience; hospice, critical care, med‑surg, ICU or ED preferred.
• Ability to work independently in the field and manage crisis situations.

Responsibilities

• Provide after-hours nursing coverage for hospice patients and families in the home setting.
• Triage incoming calls from patients, caregivers, and facilities, prioritizing based on urgency.
• Respond promptly to clinical concerns and provide guidance or interventions as appropriate.
• Perform emergency and unscheduled home visits when needed.
• Assess and manage pain and other end-of-life symptoms using appropriate clinical interventions.
• Communicate changes in patient condition to the physician and interdisciplinary team.
• Ensure continuity of care by following established care plans and hospice protocols.
• Accurately document all patient interactions, assessments, and interventions per agency policy.
• Maintain availability and responsiveness during scheduled on-call shifts.
• Proficiency in using electronic health record (EHR) systems for documentation and communication.
• Participate in hand-off communication with the daytime care team.
