Job summary

All Care VNA, Hospice & Private Home Care Services seeks an On-Call Hospice RN to provide after-hours nursing support in the home setting for patients in Boston and surrounding communities. You will triage calls, manage urgent symptoms, and visit as needed to ensure continuity of care per the patient’s plan and hospice standards.

Ideal candidates have MA RN licensure, 1–2 years of clinical hospice or critical care experience, strong assessment skills, and the ability to work independently in the

Qualifications

• Valid MA RN license required.
• 1-2 years of recent clinical experience, hospice/critical care preferred.
• Ability to independently conduct home visits and triage urgent symptoms.
• Strong assessment, communication, and crisis-support skills.

Responsibilities

• Provide after-hours nursing coverage for hospice patients and families in the home setting.
• Triage incoming calls from patients, caregivers, and facilities, prioritizing urgency.
• Respond to clinical concerns and provide appropriate guidance or interventions.
• Perform emergency and unscheduled home visits as needed.
• Assess and manage pain and end-of-life symptoms using clinical interventions.
• Communicate changes in patient condition to physician and care team.
• Ensure continuity of care per care plans and hospice protocols.
• Document interactions, assessments, and interventions per policy.
• Maintain availability during scheduled on-call shifts and use of EHR for documentation.
