Job summary

Lakes Region VNA and Hospice in Meredith, NH is seeking an On Call RN to support after-hours hospice and home health patients. The RN will triage calls, perform visits in patients' homes, and provide compassionate end-of-life care as needed.

Position is part-time, Sundays through Thursdays 4pm–8pm and Monday–Thursday 5am–8am. A BSN preferred, NH RN license required, with strong communication and time-management skills and reliable transportation.

Qualifications

• BSN preferred.
• NH RN license/currently licensed.
• Experience in home health care and hospice is preferred.

Responsibilities

• Triage patient calls and provide appropriate guidance.
• Conduct home visits to assess and address patient needs.
• Collaborate with interdisciplinary team for comprehensive care.
• Document patient interactions accurately and timely.
• Provide emotional support and education to patients and families.
• Deliver end-of-life care and support for patients and families.
• Adhere to infection control protocols and safety guidelines.
• Participate in team meetings and ongoing training.
