Beacon Hospice, Amedisys Optum is seeking a Hospice On-Call Registered Nurse for after-hours care. The role involves 7 on/off shifts, remote and field-based nursing, and triage to ensure continuous, high-quality hospice care at home.

You will assess patients, coordinate with physicians and care teams, educate families on symptom management, and document clinical decisions. A valid RN license and CPR certification are required.

Competenze

• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• 1+ years of nursing experience, preferably in hospice, home health, or acute care.

Mansioni

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions.
• Assess patient conditions via phone and/or in-person visits and escalated care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
