Job summary

Job description

The On-Call Hospice RN provides after-hours nursing support to hospice patients and their families in the home setting. This role involves triaging calls, managing urgent symptoms, making visits as needed, and ensuring continuity of care in accordance with the patient’s plan of care and hospice standards.

Schedule

Sunday, Monday, Tuesday and Wednesday | 5pm-8am | 15 hour shift

Work Location

On the road in assigned territory, including Boston and surrounding communities.

Essential Duties & Responsibilities

• Provide after-hours nursing coverage for hospice patients and families in the home setting
• Triage incoming calls from patients, caregivers, and facilities, prioritizing based on urgency
• Respond promptly to clinical concerns and provide guidance or interventions as appropriate
• Perform emergency and unscheduled home visits when needed
• Assess and manage pain and other end-of-life symptoms using appropriate clinical interventions
• Communicate changes in patient condition to the physician and interdisciplinary team
• Ensure continuity of care by following established care plans and hospice protocols
• Accurately document all patient interactions, assessments, and interventions per agency policy
• Maintain availability and responsiveness during scheduled on-call shifts
• Proficiency in using electronic health record (EHR) systems for documentation and communication
• Participate in hand-off communication with the daytime care team

Qualifications

• Valid driver’s license and reliable transportation
• Active Massachusetts Registered Nurse (RN) license is required
• 1-2 years of recent clinical experience required (hospice, critical care, med‑surg, ICU or ED strongly preferred)
• Strong clinical assessment and symptom‑management skills
• Ability to work independently in the field
• Compassionate communication and crisis‑support skills
