Job summary

Apploi is seeking a Hospice On Call RN to respond to after-hours patient census and family inquiries. The role provides urgent clinical and psychosocial interventions, pain and symptom management, and collaboration with the Hospice team to optimize patient outcomes.

Must maintain confidentiality and documentation, and drive care with nurses and physicians as needed. The position emphasizes independent assessment in crisis situations, with opportunities for professional growth and participation

Qualifications

• Graduate of an accredited nursing program with active RN license.
• Experience in hospice, homecare, hospital med-surge or oncology preferred.
• Ability to work independently and in crisis situations.

Responsibilities

• Respond to after-hours census and referrals by phone and email.
• Provide urgent clinical and psychosocial interventions and pain control.
• Collaborate with physicians and IDG for continuity of care.
• Complete and document patient care and medication changes promptly.
• Maintain patient safety and communicate changes to the team.
