Job summary

UnitedHealth Group is seeking a compassionate Registered Nurse for Hospice On-Call (Mon-Thu) to support after-hours clinical needs across assigned states. The role combines remote and field-based nursing to triage, assess, and coordinate care, ensuring continuity and high-quality outcomes for patients at home.

Responsibilities include on-call triage, coordinating with physicians and care teams, educating patients and caregivers, and maintaining accurate documentation.

Qualifications

• Current RN license for the state(s) you are assigned to work.
• Valid driver's license, reliable transportation, and liability insurance.
• Current CPR certification.
• 1+ years of nursing experience, preferably in hospice, home health, or acute care.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions
• Assess patient conditions via phone and/or in-person visits and escalate care as needed
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery
• Deliver patient and caregiver education related to symptom management and care plans
• Maintain accurate and timely documentation of all patient interactions and clinical decisions
