Job summary

UnitedHealth Group, in partnership with Amedisys, is hiring an Hospice RN On-Call to provide after-hours clinical support, triage patient needs, and coordinate care to ensure continuity and quality outcomes. This role combines remote and field-based nursing to support patients and families.

Shift patterns include nights and weekends, with duties spanning assessment by phone or visit, documentation, and collaboration with physicians and care teams.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• 1+ year of nursing experience, preferably in hospice, home health, or acute care
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions
• Assess patient conditions via phone and/or in-person visits and elevate care as needed
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery
• Deliver patient and caregiver education related to symptom management and care plans
• Maintain accurate and timely documentation of all patient interactions and clinical decisions
