Job summary

UnitedHealth Group is seeking a Hospice On-Call Registered Nurse to provide after-hours clinical support, triage needs, and coordinate care for continuity and quality outcomes. The role combines remote and field nursing, with a 7 days on, 7 days off schedule.

You will assess patients, collaborate with interdisciplinary teams, and educate families while maintaining detailed documentation. A strong hospice/home health background is preferred.

Qualifications

• Active RN license for the assigned state(s).
• Current CPR certification.
• 1+ year of hospice, home health, or acute care nursing.
• Valid driver's license, dependable transportation and liability insurance.

Responsibilities

• Provide after-hours on-call nursing support and triage patient needs.
• Assess patient conditions via phone or in-person visits and escalate as needed.
• Coordinate with physicians, care teams, and caregivers for timely care.
• Educate patients and caregivers on symptom management and care plans.
• Document all patient encounters and clinical decisions accurately.
