Job summary

Heartland Hospice is seeking an After Hours / On-Call Registered Nurse to deliver compassionate care in patients’ homes during evenings, nights, weekends, and holidays. You’ll liaise with families, physicians, and the hospice team, manage urgent calls, and guide the plan of care in collaboration with interdisciplinary staff.

The role requires an active RN license, CPR certification, and 1+ year of nursing experience, with comfort in home-based settings and reliable transportation.

Responsibilities

• Provide direct patient care and support after hours
• Serve as primary liaison between patients, families, physicians, and the hospice team
• Respond promptly to urgent calls and symptom management needs
• Document visits and communications accurately in the EMR
• Participate in quality assessment and improvement programs
