Job summary

Heartland Hospice is seeking an experienced After Hours / On-Call Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care in patients' homes or facilities. You will coordinate with the daytime team, document care, and participate in improving quality and outcomes.

The ideal candidate holds an active RN license in the appropriate state, has 1+ year of nursing experience, CPR, and a calm, compassionate approach to urgent care and family

Qualifications

• Current RN license in the state of requested employment (or eligibility to obtain).
• 1+ years of nursing experience.
• Compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment and the ability to provide support in emotionally intense settings.
• Comfortable providing care in patients’ homes, facilities, or wherever they reside.
• Valid driver’s license, automobile insurance, and reliable transportation.
• Current CPR certification.

Responsibilities

• Respond to patient needs after hours and on-call.
• Serve as liaison between patients, families, physicians, and the hospice team during shift.
• Evaluate patient and family needs and implement the plan of care.
• Document visits and communications accurately.
• Collaborate with interdisciplinary group (LPNs/LVNs, CNAs).
• Maintain infection control and safety standards.
