Heartland Hospice is seeking an After Hours / On-Call RN to provide compassionate, high-quality care in patients' homes after hours, including evenings, nights, weekends, and holidays. You will collaborate with the interdisciplinary team to adjust plans of care and ensure seamless transition with daytime staff.

The role offers a $7,500 sign-on bonus and requires MO RN licensure, CPR, and a valid driver's license.

Qualifications

• Current RN license in MO.
• 1+ years nursing experience.
• CPR certification.
• Valid driver’s license and reliable transportation.

Responsabilités

• Provide direct patient care after normal business hours.
• Serve as liaison between patients, families, physicians, and hospice team during shift.
• Respond promptly to urgent calls and manage symptoms per plan of care.
• Document visits and communications in EMR.
• Participate in quality assessment and performance improvement programs.
