Job summary

Heartland Hospice is seeking an After Hours / On-Call Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays. You will work with a multidisciplinary team to deliver compassionate, high-quality care in patients' homes or residence settings.

Eligible candidates have an active RN license, 1+ year of nursing experience, CPR certification, and reliable transportation. A $7,500 sign-on bonus is available for qualifying applicants.

Qualifications

• Current RN license in the state of employment.
• 1+ years of nursing experience.
• CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct patient care after normal business hours and document visits.
• Serve as the primary liaison between patients, families, physicians, and the hospice team.
• Respond promptly to urgent calls, assess needs, and implement the plan of care.
• Maintain accurate medical records and participate in quality improvement initiatives.
