Job summary

Metropolitan Jewish Health System in New York seeks an experienced RN for our hospice and palliative care after-hours on-call role. You will respond to urgent patient needs, coordinate with the triage team and hospice physician, and implement the care plan including medication safety and symptom management.

Graduate from an accredited School of Nursing with NY RN licensure; CHPN certification within one year is encouraged.

Qualifications

• Two years experience in Medical-Surgical Nursing, Community Health, Hospice or Oncology nursing preferred.
• New York State RN licensure required; CHPN certification encouraged within the first year.

Responsibilities

• Provide on-call patient visits scheduled, or unscheduled, for urgent physical and psycho-social issues during after-hours shifts.
• Coordinate with Call Center Triage and On Call Manager; respond via cell phone and communicate with the on-call team and Hospice physician.
• Implement the plan of care; educate patient and primary caregiver on changes, medication management, safety and symptom management.
• Attend patient deaths to provide emotional support and guidance to family.
• Report off to triage and primary Hospice team concerning visits, plan changes, and follow-up needs.
• Follow NYS Standards for Professional Registered Nurse.
