One Community Hospice seeks an On-Call RN to respond to patient census and referrals after hours. The role provides urgent clinical and psychosocial interventions, prioritizing pain and symptom management and collaboration with the hospice team and external providers.

The RN visits patients at home, communicates promptly with physicians, and documents care thoroughly. A Florida-licensed RN with hospice/homecare experience is preferred, with an emphasis on independent judgement during crises.

Qualifikationen

• Graduate of an accredited school of nursing.
• Current unencumbered nursing license in the applicable state.
• Experience in hospice, homecare, or medical-surgical settings preferred.
• Ability to perform comprehensive assessments after hours.
• Knowledge of pain and symptom management principles.

Aufgaben

• Be consistently available by phone and email during assigned shifts.
• Conduct urgent physical and psychosocial assessments after hours.
• Maintain a safe environment and provide skilled nursing care.
• Manage pain and symptoms with appropriate interventions.
• Provide guidance on diagnosis, medications, and progress.
• Document care thoroughly and timely in charts.
• Coordinate with physicians and IDG for continuity of care.
• Arrange after-hours DME, supplies, and medications as needed.
• Participate in mandatory meetings and ongoing education.
