Job summary

Reserve Care is seeking an On-Call Nurse to support patients and families in home settings with after-hours nursing care. The role emphasizes autonomy, compassion, and collaboration with physicians to manage symptoms and provide education.

The RN will assess needs, deliver intermittent skilled care under physician direction, and maintain documentation while participating in team meetings and triage as needed.

Qualifications

• Graduate of an accredited school of nursing and an Ohio RN license.
• Bachelor’s degree preferred.
• Experience in home care or end-of-life care preferred.

Responsibilities

• Assess patient and family needs in home settings (RN).
• Provide intermittent skilled nursing services under physician direction.
• Monitor pain and symptoms; support patient and family.
• Coordinate with physician and team; maintain documentation.
• Deliver education to patient and family about care at home.
